I am participating in a Meditation + Breathwork Training offered by (Mandy Trapp of Lifestyle Meditation) during which I will receive information and instruction about breathwork and meditation and will participate in a practical application of these practices during this training. In consideration of being permitted to participate in the Workshop, I agree to assume full responsibility for any risks, injuries or damages, known or unknown, which I might incur as a result of participating in the program. In further consideration of being permitted to participate in the session, I knowingly, voluntarily and expressly waive any claim I may have against (Mandy Trapp and Lifestyle Meditation) for injury or damages that I may sustain as a result of participating in the breathwork and meditation training.

I understand there are shared accommodations and availability is based on first-come, first-served registration.

I understand there is a $600 CAD non-refundable deposit required to book my spot.
